    Diabetes Mellitus

    Diabetes mellitus may be diagnosed if any of the following is present.

    1. Casual plasma glucose =/> 11.1mmol/L
    2. Fasting plasma glucose =/> 7.0mmol/L
    3. 2-hour post prandial plasma glucose =/> 11.1mmol/L

    Pre-diabetes is defined by glucose levels that are higher than normal but lower than diabetes thresholds.  Pre-diabetes states include impaired fasting glucose (IFG) and impaired glucose tolerance (IGT).

    Pre-diabetes Fasting plasma glucose (mmol/L) 2-hr post-prandial glucose (mmol/L)
    IFG 6.1 – 6.9 < 7.8
    IGT < 7.0 7.8 – 11.0

    Chronic diabetes is associated with long-term complications resulting from damages to various organ and tissues, especially the kidneys, eyes, nerves, heart and blood vessels.

    6 Eye assessment Annually
    7 Foot assessment Annually
    8 Smoking assessment Annually
    9 Cardiac assessment – ECG / others At baseline & when indicated

    Pre-diabetes

    1 Screen for CVD risk factors – BP / lipids Annually
    2 Blood glucose test-FBG, OGTT, HBA1C 6 monthly
    3 Weight & BMI assessment 6 monthly
    4 Renal function Annually
